Your personal information is used solely for the purpose of scheduling and conducting pulmonary function tests, communicating with you regarding your appointments and results, processing billing and insurance claims, and ensuring quality service delivery.

Cancellation Policy:
Appointments must be cancelled at least 24 hours prior to the scheduled appointment time to receive a refund of any deposit paid. Deposits are non-refundable for cancellations made within 24 hours of the appointment time.
No Show Policy:
If our technician arrives at the scheduled location for the appointment and the customer is not present, the customer's card will be charged a $50 no-show fee. This fee will be applied, your appointment will be canceled, and your non-refundable deposit will be forfeited if the technician has to wait for more than a hour due to the customer's absence.
Insurance Payment Responsibility:
If the patient's insurance provider does not cover the cost of the service, the patient will be responsible for the entirety of the balance for the service.
Medical Release Form Requirement:
We will not release any patient information until the Medical Release Form has been completed. This ensures compliance with privacy regulations and protects your personal information.
Prescription Order Requirement:
All appointments using insurance as payment are subject to cancellation if the prescription order is not received. This ensures that appointments are scheduled only when necessary documentation is in place.
Changes to Privacy Policy:
We reserve the right to update or modify this Privacy Policy as necessary. Any changes will be effective immediately upon posting on our website or other platforms where this policy is made available.
